Desirable:  Degree in Pharmacy from a recognised Institution/ University .Experience in dispensing and/or storage and dispensing of drugs in a reputed hospital or institution or in a drug store or a pharmaceutical concern.

Workshop (R &AL) Technician Grade-II / Gr- C / 02  Posts (UR)

Educational Qualification :  Diploma (not less than 2 years duration) in Prosthetic and Orthotic Engineering from a recognised Institution/ Board. Candidate should be registered with Rehabilitation Council of India

Pay Band & Grade Pay :  (PB-1 Rs.5200-20200 + Rs.2400 Grade Pay) 

Age Limit :  30 years 

Application Fees: The application fees - Rs.500/-  the candidate can pay the prescribed application fee through a Challan form (to be downloaded from internet while applying on-line) in any branch of State Bank of India OR through Debit  Card / Credit  Card. Application fee once remitted shall not be refunded under any circumstances.  No fee for SC/ST/PWD/Women Candidates of any community.

How to apply : Interested candidates applying the On-line Registration will be closed by 05:00 PM on 14/06/2015. The last date of closing of online registration and generated bank challan to deposit the fee through challan mode, can deposit fee in the nearest branch of State Bank of India latest by next two working days

EPF - Submission Process for Online Claims

Employees' Provident Fund Organisation, India (EPFO) is going to introduces a new system to facilitate online submission of transfer claims by Members with an objective to make the transfer process transparent, efficient and comfortable for your employees. A member has an option to submit his claim either through his present employer or the previous one.  You can see all such claim requests with ease, verify/correct member details, approve and submit the requests online through this portal. For online submission of the claims, the Digital signature (Class II or above) of the authorized person is required.

User name and password for this portal is same which is used in ECR portal by the establishments and accordingly may be used with high diligence.

Know your EPF Balance 2012

To facilitate  the Revised Transfer Claim form for EPFO beneficiaries marking this as first step towards launch of online Transfer Claim facilities EPFO issued revised claim form .

Salient features of  revised claim /transfer form 

• The form will be called Transfer Claim Form instead of Form 13 for easy comprehension by the beneficiaries. 
• The form can be presented after verification, either through the present employer or previous employer. 
• Earlier the form could be submitted after verification only through the present employer. 
• This form can be submitted online as well as in physical form. The facility of online submission of this form will be given shortly after process of collecting the digital signature of the employer is completed. 
• The facility to file physical form shall continue to cater to the needs of working class who do not have internet access. 
• Employee will be allowed to submit their applications online, if their employer is having registered digital signature.
• Online submission of form will introduce paperless process for claim settlement. 
•  Every beneficiary will be informed through SMS and e-mail about the stage of process of the claim to make the entire process transparent and accountable. 
• After introduction of online claim settlement, endeavor will be made to substantially reduce the assured time of settlement of transfer claim which presently is 30 days.

Revised List of Private Hospitals Empanelment for Government Employees/Officers/Pensioners/Their Dependents

Haryana Government is providing Medical Reimbursement facility to its Employees/ Pensioners and their dependents. The treatment taken from any Government Medical is fully reimbursable. The State has provided this facility for some Privates Hospitals also. These Hospitals are provided validity for 2 Years. After 2 years the State Government reviews the performance and track record of these Private Hospitals and again issues a New list of Hospitals from which the Employees/Pensioners and their dependents can take treatment. The Rate of Reimbursement of Medical Bills from Private Hospitals are the PGI Rates + 75% of the Balance amount. All empanelled Hospitals shall provide treatment to the employees in the event of a life threatening emergency without asking for deposition of the cost of treatment in Advance. The patient shall pay the dues before discharge from the Hospital. The Hospital, however, shall verify the identity of the employee or his/her dependent to avoid impersonation. 

The State Government, Health Department, Haryana, has issued a notification in this regard vide letter no. 2/26/2013-1/H.B-III Dated 24.06.2013. The Hospitals mentioned in the list will remain empanelled for period of 2 years at a time which can be renewed further by the Government keeping in view the performance and track record of the Hospital. The period of 2 years will be determined from the date i.e. 02-05-2013. 

The instructions dated 02-05-2011 (except Annexure regarding rates of implants) and Pars-3(a) & 3(b) as well as annexure-A and B of the instruction dated 06-05-2005 shall stand superseded.
